MEDIF (Medical Information Form) is a medical form required by airlines for passengers who require special care or assistance during the flight due to their medical condition. It is completed by the passenger and the attending physician and provides information about the traveller's medical condition, medical recommendations and any needs during the trip.

Submit the MEDIF form to us no later than 72 hours before departure. If you have purchased your ticket through an intermediary/agency, please contact them to submit the MEDIF form.

Requirements for flights to/from the USA

The MEDIF medical certificate must be issued less than 10 days before the planned date of the first flight.

What does this mean in practice? For example, a passenger booking a flight from Warsaw to New York with departure on 15 January and return on 15 April must present a medical certificate issued on or after 5 January. There is no need to present another medical certificate issued on or after 5 April.

Requirements for flights to other countries

The MEDIF medical certificate must be issued less than 14 days before the scheduled flight date.

This means that the same MEDIF form is valid for the return journey if the return flight date is scheduled within 14 days of the MEDIF issue date. If the return flight is scheduled later than 14 days after the MEDIF is issued, a new document is required for the return flight.

When a MEDIF form should be filled in?

Passengers travelling with an oxygen concentrator or other specialised medical equipment

Please complete and submit a MEDIF form if you need to use your own oxygen concentrator or other specialised medical equipment during the flight.There is no associated charge or fee.

Service of transporting your own or being provided with an oxygen cylinder is unavailable.

Pregnant women

A medical form is always required for travel above the 32nd week of pregnancy.

The form is only required below the 32nd week of pregnancy if the pregnancy has complications and/or is a multiple pregnancy.

Passengers travelling with infant younger than 7 days

Passengers travelling with infants younger than 7 days are required to fill in a MEDIF form.

NOTE: Incubators are not allowed on board our aeroplanes.

Passengers requiring the assistance of a doctor or medical personnel during the flight

Complete and submit the MEDIF form to inform our cabin crew, who are responsible for the safety of all passengers throughout the flight.

Passengers whose health might deteriorate as a result of air travel

In particular, passengers with:

  • severe anemia,
  • psychiatric disorder,
  • seizure disorder,
  • cardiac condition (e.g. cardiac failure, recently experienced heart attack),
  • respiratory problems,
  • bladder control problem,
  • bowel control problem,
  • health deterioration after recent surgeries.

Don't risk it! Be sure to consult with a doctor to confirm that you can safely travel by plane. Depending on the type of surgery or procedure, prepare the MEDIF form, which may be helpful in the case of unforeseen events or symptoms. Remember, your safety is uppermost. 

Who does not need to complete the MEDIF medical form?

  • Blind or visually impaired persons.
  • Deaf persons.
  • Persons with intellectual or developmental disabilities requiring assistance at the airport, when boarding/deboarding an aircraft (DPNA).
  • Elderly persons.
  • Pregnant women without complications up to the full 32nd week,
  • Passengers with limited mobility who require a wheelchair for the comfort of their journey or who move with the aid of a wheelchair crutch, cane or walker.
  • Passengers who need to use a sleep therapy device/continuous positive airway pressure machines (CPAP) during the flight as long as there are no other health complications/problems, qualify the passenger to provide a medical certificate.
  • Passengers with a plaster bandage, provided it has been more than 48h since the plaster was applied.
  • Diabetics travelling with an insulin pump or self-administering subcutaneous injections with PEN-type products.

Before completing the MEDIF form, please read our guidance notes

  • Carefully read all the questions on the form and answer each one.
  • Complete the first page (Appendix A) yourself. The remaining two pages (Appendix B) are to be completed by the attending physician.
  • Use capital letters, please write clearly and do not skip any question.
  • Please provide all required signatures (including on the first page). An incomplete form will not be processed.

Remember!

  • If you are travelling with a second person who will be assisting you, do not forget to provide their details and booking number (if different from yours).
  • LOT does not offer the option of travelling on a stretcher or in a fully reclined position. 
  • Read what services for people with disabilities are available on board so that you know what assistance you can use at each stage of your journey.
  • If you are travelling with your own wheelchair, do not forget to specify the exact parameters of the equipment.
  • When carrying battery-powered equipment, remember that there is no possibility of power from the on-board socket, so you must have extra batteries to last 150% of the duration of the flight. 
  • There is no access to an oxygen cylinder on board. 
  • Our cabin crew are only trained to administer first aid. They are not authorised to give injections, administer medication or lift or carry passengers.
  • If your health has deteriorated between the issuing of the MEDIF form and your journey, a new medical certificate must be prepared. 
  • The MEDIF certificate is only valid for the flights and dates specified in this document.
  • Please note that a copy of the MEDIF document must always be presented to: the staff at the airport, at check-in and to the cabin crew before boarding.
