I confirm that my dog is trained to behave properly in public and takes direction upon my command.

I confirm that the size of my dog allows to easily accommodate it within my foot space, under the seat in front of me or in my lap, without protruding the foot space of adjacent passengers or certain other areas that must remain unobstructed to comply with safety regulations.

I confirm that my dog will remain on a leash or a harness for the entire flight.

I assume full responsibility for the behavior of my dog while on the plane, including its interactions with crew, other passengers and their property. I am aware that I will be obligated to reimburse LOT Polish Airlines or its passengers for any loss, damage or expense resulting from any misbehavior of my dog.

I am aware that there is no designated place aboard where an animal can relieve itself, thereby I confirm that my dog will not need to relieve itself, or it will do so in a way that does not cause health risk to others or hygiene problems.

I am aware that, if my dog acts inappropriately, LOT Polish Airlines may refuse to carry my dog aboard as a service dog.

I declare that I have read the Conditions of Carriage for Certified Assistance Dogs by LOT Polish Airlines and I confirm that the assistance dog meets all the requirements for transport as an assistance animal (SVAN), and therefore I am aware of the legal consequences of certifying untruth.
